We require payment in full for your procedure on the day your procedure is performed. We accept Visa, MasterCard, American Express, cash, and checks. If you are interested in a financing option, we offer third party financing, with an interest free revolving charge designed to meet your treatment plan needs on approved credit.
A specific amount of time is reserved especially for you and we strongly encourage all patients to keep their appointments. If you must change your appointment, we require at least 48-hour notice to avoid a $50/hour cancellation fee (personal emergencies are an exception).
In the event of a dental emergency after regular business hours a $95 emergency fee will be charged for established patients, in addition to the necessary treatment fees.
If you would like us to submit a claim to your dental insurance company on your behalf, please read each item below:
Your dental plan is based upon a legal contract made between your employer and an insurance company. If you have any questions regarding the limitations of your dental plan please contact your employer or insurance company directly. While dental insurance is a valuable stipend for some patients, it is often not intended to cover all of your dental care costs.
If requested, we will submit a claim to your insurance carrier, as a courtesy. Once we receive payment from your insurance carrier, we will immediately send you a check for that amount. Our office is not, and cannot be, part of your legal contract with your insurance carrier. Ultimately, you are responsible for all charges in our office.
We currently can submit claims to all private dental care insurance plans. This means that we work with literally hundreds of companies. The insurance information (i.e., limitations, coverage, remaining allowance, etc.) to which our office has access, might not reflect your coverage in real time. Therefore, it is impossible to give you a guaranteed quote at the time of service. We can call your insurance carrier and obtain an estimate of your out-of-pocket expense. But it is only an estimate. If you would like to know your insurance benefit, we will be happy to file a “pre-treatment authorization” with your insurance carrier prior to treatment. Keep in mind: this is not a guarantee of coverage.
I have read the above conditions and understand that I am responsible for all costs for treatment performed at First Hill Dental Implant Center (FHDIC) on the day of treatment. If I have chosen to have FHDIC submit a claim to my insurance carrier for my treatment, I understand that FHDIC will reimburse me for the amount paid by my insurance carrier.
